Welcome to Fitness Therapy Hawaii!
Signing up for our fitness gym and classes is similar to booking a flight or restaurant reservation. Our “by-appointment-only” approach makes us a sought-after fitness experience. The online membership app will allow you to schedule individual sessions or group classes and make payments using the GymMaster Mobile App (for Android or iOS) or through the online member portal.
HERE’S WHAT YOU NEED TO DO:

STEP 1: NEW MEMBERSHIP
Our program requires all participants to provide a Physician’s Clearance Form to get started.
All participants must be cleared by their physician and complete the registration details listed in Step 2 to schedule a consultation and gym tour.
Download the Physician Clearance Form from our website and give it to your physician to complete for you. Your physician should fax the completed form to FTH at +1 (808) 215-8989 or email the form to georgema@fitnesstherapyhawaii.com

STEP 2: COMPLETE REGISTRATION DETAILS
Log into our member portal: https://fitnesstherapyhawaii.gymmasteronline.com/portal
and click “Enquiry” and complete the registration form.
Once your Physician’s Clearance Form has been received from your physician and reviewed by Fitness Therapy Hawaii, we will contact you using the contact information you provided in your registration.
We can then schedule your consultation and a tour of our facility.
You will meet with an FTH team member (virtual or in person) for a fitness assessment to determine and establish a baseline ability zone.
Participants are also required to complete an intake process, which includes an Exercise Pre-Screening Questionnaire, Informed Consent & Waiver and Release of Liability Form, Parkinson’s PDQ-39 Questionnaire, and an in-person physical assessment.
STEP 3: HOW TO BOOK A SERVICE
Once your pre-participation forms have been completed and signed, you can download the free GYM Master Member APP on your mobile device:

Sign in with your details. Your “User Name” is the email to which we send your notifications.
If this is your first time using the app, click the “Forget password; to Reset Password” link and retrieve the email that will be sent to you and follow the password reset instructions.
Once you successfully sign on to the GymMaster App, you will be able to:
View and private book sessions
Book into group classes and fitness gym hours
Mobile door access - RFID scanner that allows gym members entry into the club with their phone
Manage your bookings
Update your personal information
Update your billing information
View your visit history
Access the PD exercise library
Record and track your measurements and fitness goals
Receive push notifications from us with reminders and promotions
STEP 4: SERVICE MENU
To purchase a membership, program, or book sessions from our website, go back to Step 2 and log in to the member portal:
Select “Sign Up” and choose a program from the menu of available programs. Next, select the membership type you would like to enroll in.
You must register to attend any Fitness Classes & Training Services. Class sizes are limited. Credit cards or bank auto-debits are required for monthly membership payments.
Note: You must log in to the Member Portal to select your program and then GymMaster Member App to schedule your sessions.
